Cardholder Forms

• The electronic PCard Application is accessible via myUFL. Instruction guides are available in the UF GO Expense Toolkit.

• The PDF UF GO PCard Application will still be accepted through 9/30/2024

Transaction Support Forms

• The Missing Receipt Declaration is a built-in UF GO form that should be used in lieu of the old Replacement Receipt Form. Instructions are available in the UF GO Expense Toolkit: Missing Receipts.
• UF Sales Tax Exemption Certificate
• PCard Requisition Form (.pdf file)
  To improve internal departmental controls the University’s Office of Internal Audit recommends use of a PCard requisition form. Departments are encouraged to incorporate this form into their PCard processes.
• Vendor Agreement for Purchase of Travel Services
